Position description – Children’s Hospital New Orleans is currently seeking ED Clinical Pharmacists for expansion of services to a freestanding ED as well as main campus. The ED Clinical Pharmacists will work as part of the multidisciplinary team to deliver safe, appropriate, comprehensive, and cost-effective medication therapy.
Job responsibilities include the following:
Review drug therapies and apply appropriate pharmacokinetic and pharmacodynamic principles to provide optimal drug therapy recommendations
Serve as a drug information resource for providers, nurses, fellow pharmacists, and other healthcare providers
Interview and counsel patients and/or families regarding safe and effective medication use
Collaborate with the multidisciplinary team to enhance patient outcomes
Develop and implement clinical practice guidelines to enhance pharmaceutical care and medication safety
Participate in medication-use evaluations and formulary management
Detect and document adverse drug reactions, clinical interventions, and medication errors
Required experience – Doctor of Pharmacy from an accredited college of pharmacy; PGY1 Pharmacy Residency and/or 3+ years of experience. PGY2 Pharmacy Residency OR PGY1 Pharmacy Residency with 2-4 years of hospital pharmacist practice experience is preferred; current licensure or eligible for licensure by the Louisiana Board of Pharmacy
